Hi I have a few more questions from case study exam 6.
For case 7: Questions 27-31
Coen was born at 42 weeks and had a seizure within the first 24 hours of birth and 6-12 Petit Mal (Absence) seizures within the first 3 months of birth.
Patient Information:
in hospital during first two weeks after birth, an ECG indicated edema of the brain and an MRI revealed moderate damage to the cortex and brain stem
upon returning home with parents, he was unable to nurse, having become accustomed to a feeding tube
upon the use of a nipple shield, Coen started breastfeeding
over the next 3 months, Coen appeared fine, with good reflexes, muscle tone, and a bright spirit
after 3 months, he began to have seizures with 10 recorded during a 24-hour ECG
the doctor prescribed Phenobarbital for 8 days in a row. During this, Coen became irritable and cried easily. The seizures increased to 15/day after the medication.
after this, Coen became unable to hold his head up longer than a few minutes while resting on his belly, and if tired, could not hold his head up for more than a minute
it became evident that Coen was not nursing very often or long – he was neither gaining weight nor growing at the expected rate for his age
he had a weak appetite and limited capacity for food
squirming hands and feet
frequent loose stool
pale urine
pale face
Pulse: Weak (Ruo)
Tongue: pale
I wasn’t sure which pattern to chose in Q28 as I was tossing up whether there was more blood def or LV qi stagnation. In the end I went with blood def because of all the pale signs and because of the Q31. Which is why I am confused about the following answers:
Q28: What is the TCM pattern
The correct answers are Liver Wind, and Liver Qi Stagnation and Kidney and Spleen Qi Deficiency with Liver Wind. So i got this wrong but then I got the rest of the questions right even though I based them all of my answer to Q 28.
But then for Q29 the correct treatment principle is tonify Spleen, Kidney and Stomach Qi; tonify Blood
These don’t seem to line up as I would think “move Liver Qi; Tonify Kidney and Spleen Qi; expel Wind” would be a better suited answer if you chose correctly in q 28.
Can you please explain this?
It gets more confusing as in Q30 Which of the following points may be unnecessary in treatment?
The answer is GB 31 which would help with wind so it doesn’t make sense to me why you wouldn’t use this point if the answer for Q28 had LV wind in twice. But it would make sense if your treatment principle didnt include wind.
Then question 31: what would you do if there was still convulsion, the answer is expel wind. Which again makes sense based on the treatment principle as wind wasnt in there but doesn’t match the original pattern.
